New Cases:
New cases will be seen only by prior appointment. The
case will be taken by Dr. Shankar Chawla or by the
assistant doctor but will be prescribed only after
consulting with Dr. Shankar Chawla.
In order to ensure quality service, we have to limit the
number of new cases.
If you wish to take a new appointment, you should leave
your name and telephone number at the reception. If it
is possible for us to accommodate the case, we will
certainly do so.
Follow-ups:
Follow-ups will be seen by Dr. Shankar Chawla from
Monday to Saturday at Clinic I from 10:00 a.m. to 1:00 p.m.
except wednesday and at Clinic II from
5:00 p.m. to 8:30 p.m. except thursday. Follow ups will be
seen only by appointments. For any problems or acutes in
between please call us on the phone at the clinic and
speak with one of the doctors and follow the advice.
We suggest that you book your next follow up when you
visit the doctor. Or phone at least three weeks in
advance to book a follow up since appointments get
booked fairly soon .
In case you have not done so, and need a follow up you
can choose to take one of the following options:
1) You have the facility to give your follow-up over the
phone or email. You can phone the clinic and leave a
message at the reception (98191 43626). The doctor will
get back to you when he/she is free.
2) You can also email your follow up on health@askdrchawla.com.
However, if the doctor is not satisfied with your
phone/email follow up, you will be required to visit the
clinic. We recommend that patients should not use the
phone for follow up facility for a prolonged time and
should visit the clinic once in every two or three
months.
Your follow up report (whether in person or by
phone/email) should be as per the following:
What you need to report on follow up
1) How are the complaints you came with.
2) Any new complaints to report.
3) How has been your mood and state of mind.
4) Any situation that has affected you or is causing
stress.
5) Did you notice any change in the way you are dealing
with stressful situations.
6) What dreams did you have.
7) How are you feeling in general.
8) Any test / report you have done.
9) Any thing else you like to say or report, or any
queries you may have.
In case you have not come personally for the follow
up, medicines can be couriered to you or they can be
picked up from the clinic.
